2. Complete entire application, answering all the questions. If a question is not applicable to you, put n/a. Please answer the four additional questions in a one-page essay on a separate piece of paper.
3. Include a copy of your High School Diploma or GED certificate. If you are a college graduate, please include a copy of your Diploma or transcript.
4. Sign the form and return it to us with your $100 nonrefundable application fee.

Enrollment
Following the successful completion of the interview, if required, and review of all application requirements, the student will receive an Enrollment Agreement. Registration in either the Energy Medicine Certificate Program or the Energy Medicine Practitioner Program is complete when IHAS receives a signed Enrollment Agreement and tuition has been paid in full.
